Your First Visit
When you called an made your initial appointment we sent a Registration & Medical History form. Please complete the form and bring it with you as well as your dental insurance card or information.
We know that dental visits can be anxious times, so we will start with a general review of the information you have provided.
Dr. Chann will review your medical history and perform an oral exam explaining and diagnoses and treatment options. During this time you will be given ample opportunity to discuss any concerns.
Please alert our office staff if you have any pre-existing medical concerns or conditions that might impact dental treatment. A complete listing of your medications is always helpful. It is especially important that you tell us of you require medication prior to treatment (i.e. antibiotics for pre-medication).
A parent or guardian must accompany all patients under 18 for the initial visit.
